Each partner building has a Fortis Care coordinator who keeps families informed, manages specialist referrals, and makes sure important follow-up does not get lost. They are your point of contact and advocate.

After-hours coverage—not just voicemail

Overnight and on weekends, a board-certified physician is available through telemedicine with access to the clinical record. Acute changes receive real clinical guidance in real time, not an automatic trip to the emergency department.

How is Fortis Care different from the physician group that visited before?

Many physician groups divide their time across office practices, hospitals, and different facility types. Fortis Care focuses exclusively on skilled nursing and post-acute care. Our teams are embedded in the buildings we serve, participate in daily clinical coordination, and stay closely connected to staff and residents.

Who do I call if I have a concern about my family member's care?

What happens if something changes overnight or on a weekend?

Our after-hours telemedicine service connects the nursing team with a board-certified physician who can access the medical record in real time. Acute changes can be assessed and managed in place when clinically appropriate, rather than resulting in an automatic emergency department transfer.

How often will a provider see my family member?

Visit frequency depends on clinical need. Residents with higher acuity are seen more often. Because our team is active in the building every day, it can also respond quickly when a resident's condition changes between scheduled visits.

Can I be involved in care decisions?

Absolutely. We encourage family involvement in goals-of-care conversations, discharge planning, and ongoing care discussions. Your care coordinator can help ensure you are included in the right conversations.

What if my loved one needs a specialist?

Our care coordinator manages specialist referrals and tracks follow-up so nothing gets lost. In some cases, our multi-specialty model—including palliative care and physical medicine—can address needs at the bedside without requiring a transfer.

My family member is being discharged soon. What should we know?

Transitions of care are a clinical priority. Our team coordinates with the receiving care setting—whether home health, rehabilitation, or another level of care—to help ensure the clinical picture travels with the patient. Your care coordinator can walk you through the discharge plan and what to expect.
